4 cm in length x 2.8 cm inwidth, with

（5.照片三張（最近二個月內所攝黑白或彩色脫帽未戴有色眼鏡，直四公分，橫二.八公分，正面半身薄光面紙相片。）

（八、辦妥結婚登記之戶籍謄本；無法檢附戶籍謄本者，須檢附結婚證明文件、外國籍配偶及我國籍配偶之國籍身分證明文件，於國內結婚者另繳附外國籍配偶經駐外館處認(驗)證之單身證明及中文譯本。）

※ This certificate is drovided for those applicants who,while applying for deprivation of original nationality,needstheR.O.C..tofumisha Certificate of NaturalizationofR.O.C.Nationality.Thiscertificate is submitted tothemunicipality,county(city)

government via the Household Registry for

issuance,the term of validety of this certificate

is one year.

（本項證明僅供申請人申請喪失原屬國國籍時，需要我國提供可歸化我國國籍證明者使用之；本項證明由戶政事務所轉直轄市、縣（市）政府核發，有效期限為一年。）

※If the applicant,s original cuntry does not require a certificate to verify theapplicant,s loss of nationality, it is not necessary to apply for thiscertificate.

※如原屬國無須我國提供本證明，始得喪失該國國籍者，則無須申請本證明。

（一、歸化國籍申請書(含二吋半身相片三張)。）

(2)Certificate of Statelessness or deprivation of

applicant,s original nationality, or the

documentauthenticated and issued by the

diplomatic authority inaccordance with

Article 9 of ”proviso ”of the Nationality

Law.

（二、無國籍、喪失原有外國國籍之證明文件或依國籍法第九條但書規定，由外交機關出具查證屬實之文書。）

(3)Valid Alien Resident Certificate or Valid

AlienPermanent Resident Certificate.

（三、有效外之僑居留證或外僑永久居留證。）

(4)Foreigner Resident Certificate issued by the

policeauthority.

（四、警察機關核發之外國人居留證明書。）

(5)Certificate of Entry and Exit Date issued by

theBureau of Immigration of the National

PoliceAdministration of the Ministry of the

Interior.

（五、內政部警政署入出境管理局核發之外國人入出國(境)日期證明書。）

(6)Certificate of Police Records or other

relevantcertificate issued by applicant's

original government(It the residence reason

on the Alien ResidentCertificate is

“joining family”,this requirement canBe

waivered.)

（六、原屬國政府核發之警察紀錄證明書或其他相關證明文件(其外僑居留證之居留事由為「依親」者免附)。）

residence period in the R.O.C.

(8)Certificate of certain assets or professional

skills, evidencing self-support and ensuring

a living.

（八、相當之財產或專業技能，足以自立，或生活保障無虞之證明。）

(9)Household Certificate after completing

marriageregistration.For those who cannot

attach theHousehold Certificate, please

attach Certificate ofMarriage, Certificate of

Nationality, documentsidentifying foreign

spouse or R.O.C. spouse, and, ifgetting

married in domestic areas, certificate of the

singleness of foreign spouse authenticated

by a R.O.C.overseas mission and its

Chinese version. （九、辦妥結婚登記之戶籍謄本；無法檢附戶籍謄本者，須檢附結婚證明文件、外國籍配偶及我國籍配偶之國籍身分證明文件，於國內結婚者另繳附外國籍配偶經駐外館處認(驗)證之單身證明及中文譯本。）(10)Certificate Fee：NT＄200.

deeds, inhabitancy.)

（二、戶口名簿(單獨立戶者免附，但須提憑房屋、居住等證明文件)。）

(3)Three photos (black-and-white or color full-

face andhalf-length photos, 4 cm in length

x2.8 cm in width, without wearing hat and color glasses in semi-matprinting paper,

taken within tw months.) （三、照片三張（最近二個月內所攝黑白或彩色脫帽未戴有色眼鏡，直四公分，橫二.八公分，正面半身薄光面紙相片。）

※Please apply. for Identification Card after

completing household registration.

(※申請人申報戶籍後，即可依規定請領國民身分證。)
